Medical Brochure contains all the guidelines and information required for raising a medical claim with the Trust, it is recommended to first go through the Brochure for information of the officer and his family.
Registration form is mandatory when submitting a medical claim with Trust for the first time.
Hospitalization form is required to be submitted to the Trust’s office for a treatment taken at the hospital.
Day Care Treatment form is required to be submitted to the Trust office for a treatment taken at the hospital, but discharged on the same day
Domiciliary form is require to be submitted to the Trust’s office for treatment taken at OPD, Dental and for General Treatment
